Who files FATCA / CRS
Holders of bank accounts, mutual fund folios and demat accounts. It is asked at account opening and again when something suggests the position has changed.
When
Generally at account opening, and again whenever your circumstances change — moving abroad being the obvious trigger. Institutions ask again periodically, so check with yours.
The thing people get wrong
Folios are frozen for transactions when this is outstanding, and people find out when a redemption will not go through. Tax residency is also not the same as citizenship or where you currently live, and guessing at that box is how a wrong declaration gets signed.

They are revised without notice, and a stale file served from here would be worse than none — you would file it and find out months later.
